A registered nurse visits the patient at home for a comprehensive needs assessment and to build a personalised care plan.
Your dedicated nurse or care team begins scheduled visits. All care is documented and shared with your GP and health team as needed.
Regular care plan reviews ensure the support evolves with the patient's needs. Family and whฤnau are kept informed throughout.
